How Virtual Health Monitoring can support your organisation during the COVID-19 pandemic

Virtual Health Monitoring (VHM) connects healthcare providers with patients via a secure digital health platform enabling them to monitor the health and wellbeing of their patients remotely.

VHM technologies allow patients to have their symptoms and vital signs monitored in their own home when connected to patient app. This includes monitoring pulse and oxygen saturation using a SpO2 pulse oximeter​, temperature, blood glucose, weight and blood pressure*

Using Bluetooth, data on a patients vital signs and symptoms can be either manually entered or transmitted to the MyCareManager patient app which then transmits the data securely to a web-based portal within the hospital environment. Once the data has been securely transmitted, information regarding deteriorating conditions can be automatically flagged to a clinician, enabling healthcare providers to follow up with patients by simply clicking to launch a secure video call, direct to the patient’s mobile device.

This solution is particularly relevant for those directly affected by COVID-19 (including those who are being monitored because they are at risk of contracting the virus through exposure to someone who has tested positive to the virus) as well as vulnerable populations, such as those with pre-existing chronic conditions or who are undergoing treatments that can weaken their immune systems, who need to have their health and wellbeing monitored whilst in isolation, thus reducing the risk to themselves and others who would otherwise have to monitor them in person. Our VHM solution can also be used as a resource for freeing up capacity within hospitals for patients who are more critical.

Virtual Health Monitoring consists of four modules:

Portals

A real-time dashboard of client health information including medication details, service schedules, care plans from clinical and client management systems and readings from health devices. Use the portal to set alerts to remind clients to take medications and tests. Gather data using assessments and surveys relevant to the client’s condition

Monitoring

Collects data from medical devices such as weighing scales, glucose monitors and pulse oximeters. It enables remote monitoring of chronic conditions such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), diabetes, asthma and hypertension. If readings are outside prescribed parameters, Virtual Health Monitoring can be set up to send alerts to the patient’s elected care team.

Video

High-definition video conferencing means care teams can video call clients to discuss care plan tasks, assist with medication and more. It also supports case conferencing between multiple care team members, the consumer and their family members.

MyCareManager Companion

Virtual Health Monitoring Companion, powered by Breezie, is a personalised tablet-based platform specially designed for the elderly or cognitively impaired. The interface helps remove the complexities of touch-screen technology without compromising functionality.

Cross-platform bring-your-own-device (BYOD) solution: we have listened to demand from the healthcare market to create a more affordable and accessible solution that leverages existing high rates of mobile phone ownership in Australia, reduces the overall cost of the solution and removes the burden of asset management from health services. That’s why we have created the MyCareManager patient App that can be deployed on a client’s own smartphone or tablet (both iOS and Android devices) as a bring your own device (BYOD) mobile application and a client portal that is accessible via web browser from any PC. Using a patient’s own web-enabled mobile device helps to reduce the task burden on already-stretched clinical staff as it’s one less piece of equipment they need to track and clean between patients.

Unmetered data on Telstra mobile network: any time a client accesses the MyCareManager App through a device linked to a Telstra account/SIM, the data is unmetered and they will incur no data charges - even during videoconferencing!

Data security: MyCareManager has been reviewed by, and received, Cyber Security clearances in a number of health jurisdictions. All data is hosted within the Telstra-owned data centre in Clayton, Victoria. The Telstra Cloud environment is ISO 27001 compliant.

Experience and capability to integrate with jurisdictional Telehealth networks and/or standards-based videoconferencing systems: while MyCareManager can be used as a standalone platform, Telstra Health has video engineers on staff who can rapidly collaborate with your telehealth team to integrate MyCareManager’s videoconferencing with your own telehealth network, if desired. Our video platform can also be launched from existing standards-based videoconferencing systems to allow seamless connection from any software video endpoint used by your healthcare providers.

Simplified video call experience for patients and healthcare providers: we have improved the telehealth experience for both patients and providers by doing away with the ‘telehealth invite’. Healthcare providers can simply ‘click to launch’ a secure videoconference from a variety of screens within in the provider portal of MyCareManager and all patients have to do is ‘accept’ the call.
