Challenge

With a base of about 5,000 clients across a service region of 72,000 square kilometers, clinicians and healthcare workers at Goondir Health Services needed a way to virtually monitor the health of patients with chronic illnesses.

Impact

  • Vulnerable patients with chronic conditions could access the care they need without visiting the health service, espcially during the COVID-19 outbreak.
  • Clinicians could remain confident in providing high-quality care over the telephone when required.
  • Patients felt more empowered to self-manage their own chronic conditions.

Telstra Health partnered with Goondir Aboriginal and Torres Strait Islanders Corporations for Health Services (Goondir Health Services) to help support patients on their health journey through Telstra Health’s Virtual Health Monitoring solution.

Through Telstra Health’s Virtual Health Monitoring solution, now a part of Virtual Health Platform, clinicians and healthcare workers at Goondir Health Services have been able to virtually monitor the health of patients with chronic illnesses from their comfort of their own homes.

Goondir Health Services is an Aboriginal Community Controlled Health Service (ACCHS) which provides primary healthcare and related health services to the local Aboriginal and Torres Strait Islander (ATSI) communities from Oakey in the South East of Queensland, Australia to St George in the South West of Queensland, Australia.

Goondir promotes primary healthcare services to a client base of about 5,000 clients across a service region of about 72,000 square kilometers.

CEO of Goondir Health Services, Floyd Leedie, said, “Goondir originally implemented the Virtual Health services because of COVID-19. With the restrictions put in place by the government, we found that our clients refused to come down to our clinics and a lot of the consults were done by telephone.

“Our clinicians had fears around giving good quality of care over the telephone. Therefore, it was to empower (our clinicians) to keep up the quality of care which empowered the patients to self-manage their own chronic conditions.”

Virtual Health Monitoring is provided through a secure digital health platform and enables clinicians to remotely monitor the health and wellbeing of their patients when they are connected to a mobile app via a compatible device. This typically includes monitoring of a patient’s pulse and oxygen saturation, temperature, blood glucose, weight and blood pressure.

Once the clinical data has been securely transmitted, any information regarding the patient’s conditions can be accessed by a clinician, enabling healthcare providers to quickly follow up with patients by a secure video call, direct to the patient’s compatible mobile device.

Telstra Health’s Virtual Health Monitoring solution is part of the Inreach Project, a partnership between Goondir Health Services, Darling Downs Health, UQ Centre for Online Health and Telstra Purple.

Floyd added,“By implementing the Virtual Health services we were able to include it into our recently co-designed InReach project where we’re trying to create a seamless journey between the public health system and the primary healthcare system.”
